1) Request Appointment

If you have not yet scheduled an appointment, you may do so by requesting an appointment online or by calling our office at 256-746-1300.

2) Register Online

Fill out Patient/Health Registration. When you fill this out and click submit it will send your information to us. We can also send you a link to your cell phone to fill them out as well.

3) Get Directions

If you are unable to get directions on our location page, you may call our office to get more detailed directions.
